      If you have completed Step 1 by submitting the personal history section of your application for employment to our agency, the next step is to provide the required documentation to verify your application. Please read all the instructions before you begin.

      You will be required to upload the following documents: 

      1. Social Security Card
      2. Birth Certificate
      3. High School Diploma, G.E.D., or Transcript indicating graduation
      4. College Diploma - if applicable
      5. One Full Credit Report showing account details (Available free at annualcreditreport.com)
      6. Military DD214 Long Form - if applicable
      7. P.O.S.T. Certificates - if applicable
      8. Law Enforcement Training Records - if applicable
      9. Citizenship Records - if applicable
      10. Name Change Records - if applicable
      11. Bankruptcy Discharge Records - if applicable

      It is strongly recommended that you gather electronic copies or photos all of these documents before you begin. Stopping and saving this form before submission, may remove all the  attachments you have previously uploaded and you will need to reattach each one.
